What actually decides the Premier League title race

The Premier League is still the simplest of the major leagues to follow: 20 clubs, each playing every other club home and away, three points for a win, and whoever has the most points after 38 matchweeks lifts the trophy. No play-offs, no group stage — just a single table.
Ties are broken first by goal difference, then goals scored, so late-season matches between mid-table sides can matter as much to the title race as anything at the top when a title is being chased on goal difference.
The bottom three at season's end are relegated to the Championship, which is why 'six-pointer' matches between struggling sides carry as much tension as anything at the top of the table.
The top four (sometimes five, depending on how other competitions' qualification spots shake out) usually qualify for the following season's Champions League, which is why the run-in often has as much riding on 4th place as it does on 1st.
